Cloudflare Pages Functions process the form submission, Cloudflare Turnstile helps protect the form from abuse, and configured email, hosting, security, webhook, CRM or customer-communication service providers may process the information for delivery and response purposes.
The public website form does not request Social Security numbers, dates of birth, identity documents, driver license numbers, State ID numbers, passport details, benefits-account numbers, full residential addresses, uploads, bank information or payment information. Do not include those details in the optional message field.
Submission does not guarantee Lifeline eligibility, approval, service or a specific device. Information is retained only as long as reasonably needed for application assistance, business, legal or support purposes. No internet transmission can be made perfectly secure.
